Family Care Services
Below are printable applications for each branch of Family Care Services. Please be sure that you are filling out the application for your area. You can find the parishes that each branch covers on the About Us pages.
The West Monroe branch has two applications. The Lincoln Parish application is different from all other parishes. If you live in Lincoln Parish, please make sure you are filling out the correct application.
After you fill out the application, please mail it back to the appropriate office along with a copy of your driver's license. Your application cannot be processed without it.
If you live near a Family Care office and would like to fill out an application, it is highly recommended that you come into the office to fill one out. If you come into the office, you will be able to meet with a supervisor who can discuss possible positions with you.

In order to fill out an application for Family Care Services you must:
Be 18 years of age or older
Have a High School Diploma, GED, or be a registered CNA
Have a valid Driver's License
Have dependable transportation with insurance
Have no criminal history (Parish and State Background checks will be run)
Have paid experience in caring for the elderly or handicapped (Only for certain waivers)
